the Management Team
CASE MANAGEMENT
ASSOCIATES’ professional staff consists of Registered Nurses (RN), a
Pediatric Nurse, a Certified Rehabilitation Registered Nurse (CRRN), Licensed
Rehabilitation Counselors (LRC), Certified Case Managers (CCM), IV & Home Care
Case Management Coordinators, and Paralegals.

Candace
Barone – Founder and President of CASE MANAGEMENT ASSOCIATES, INC.,
Candace has been in private sector rehabilitation since 1981, she is a Licensed
Rehabilitation Counselor (LRC), and a Certified Case Manager (CCM). Her
training and work experience includes 14 years as a Medical Assistant for a
Plastic & Reconstructive, Facial & Hand Surgeons, and an Endocrinologist. She
has a working knowledge and experience with group health reimbursement, benefit
provisions, and workers’ compensation. She has consulted with insurance
carriers and health care providers in resource development by identifying
quality, cost-efficient services, and managed care tracking systems as well as
case management program development. While certified as an Ombudsman for the
Commonwealth of Massachusetts’ Long Term Care Program, she advocated for elders
at skilled nursing facilities in the greater Boston area. Candace is Director
of our national I.V. and Home Care Case Management program and holds a
certificate in Paralegal Studies.

ADELE
POLLARD - Vice President of CASE MANAGEMENT
ASSOCIATES, INC. since 1987. Adele is a Registered Nurse who holds a Bachelor’s
Degree in Psychology, a Master of Science Degree in Health Policy and Planning,
is a Licensed Rehabilitation Counselor (LRC), and a Certified Case Manager
(CCM). Her clinical experience includes orthopedic, neurology, rehabilitation,
and psychiatric nursing in a variety of settings.
She has experience in medical legal consulting, testimony, depositions, and in
developing life care plans for adults with traumatic brain, spinal cord
injuries, and multiple trauma. Adele worked in private rehabilitation and while
employed by a Boston based accident and health carrier, she administered a
national onsite catastrophic case management program. She has consulted with
insurance carriers and managed care companies regarding psychiatric, medical and
rehabilitation case management services, telephonic UR, case management program
development, and in-house training. Adele is Director of our Onsite Case
Management program.

VIRGINIA DOWLING
- Business Manager. Virginia joined CASE
MANAGEMENT ASSOCIATES, INC. in 1995, bringing to us extensive experience in health
insurance and managed care. Virginia’s background includes developing and
coordinating the Case Management Department for a National Insurer, creating and
implementing various managed care programs, national contracting, patient
advocacy, and developing marketing and reimbursement strategies for an
international biotechnology company. She holds a certificate in Paralegal
Studies and has a clinical background, having practiced medical surgical and
home care nursing in the New England area.

MARY ANNE
DUFAULT-
Health Care Consultant.
Mary Anne joined CASE MANAGEMENT ASSOCIATES, INC. in 2002, she is a Registered
Nurse and a Certified Case Manager (CCM). In April 2002, she successfully
passed the Uniform Data Systems Credentialing Exam for FIM Practitioners. Mary
Anne comes to CMA from a fast-paced hospital environment where she was a Nurse
Case Manager. In the acute rehabilitation facility, Mary Anne, managed a
diverse patient population including the medically-complex brain injured,
neurobehavioral, and ventilator dependent cases. Her clinical background
includes medical-surgical, rehabilitation, and home care nursing.

DOTTY FOGG
- Office Manager. Dotty joined CASE MANAGEMENT ASSOCIATES, INC. in 1995.
She has extensive experience as an Office Manager in private sector
rehabilitation. Dotty is responsible for all invoicing and overseeing the
generation of all reports and correspondence relating to onsite catastrophic
case management referrals.


TAMRA MASTERSON - I.V. and Home Care Case
Manager. Tamra holds a Bachelor’s of Science Degree and joined
the CASE MANAGEMENT ASSOCIATES, INC. team in 1992. She provides telephonic
coordination, management, and negotiations of claims for our Pharmaceutical
Management Program.

NANCY
BORELLI - I.V. and Home Care Case Manager. Nancy holds a Bachelor’s
Degree in Economics. She joined the staff of CASE MANAGEMENT ASSOCIATES, INC.
in 1996. Her prior experience includes that as a Financial Coordinator for
inpatient and outpatient hospital services, as well as physician services.
Nancy also was an HMO/Blue Cross Blue Shield Team Leader for outpatient and
inpatient sites, which involved billing coordination, payment follow-up and
acted as a liaison between staff and insurers. In her current role, she
provides telephonic coordination, management, and negotiations of claims for our
Pharmaceutical Management Program.

DONNA SALAMONE
- Supervisor/I.V. and Home Care Case Manager.
Donna joined the staff of CASE MANAGEMENT ASSOCIATES, INC. in 1991. She holds
an Associate’s Degree in Business Administration and a Diploma in Nursing. Her
diverse experience in the health care field includes patient assessments for a
head injury facility and a home care rehabilitation company. Donna is Licensed
as an Accident & Health Independent Adjuster for the State of New York. In
addition to supervising the CMA I.V. and Home Care Case Managers, Donna provides
telephonic coordination, management, and negotiations of claims for our
Pharmaceutical Management Program.

ANN WHITE

Health Care Consultant. Ann
holds a Master’s Degree in the Science of Nursing, and is a Pediatric Nurse
Practitioner, she joined CASE MANAGEMENT ASSOCIATES, INC., in 1995. Ann has
clinical experience in adult and pediatric rehabilitation in a hospital, home
care, and long term care environments. Her experience also includes the
development and implementation of a Case Management program at a pediatric
hospital and rehabilitation center. In addition to managing catastrophic case
management cases, Ann’s experience includes Life Care Planning, testimony,
depositions, and medical legal nurse consulting for children and adults with
acquired brain injury, neurological disorders, and spinal cord injuries.
